Issue creating an instance in openstack via deploy pipeline

I am getting this error every time I try to deploy an instance. Error:

I am trying to deploy the next iteration of ssh proxy instance. Here are some deatils. Instance ID - 261eefc1-14be-4987-a8f5-70b0a4e0c961 Project cheaha-cloud

It was a problem with the host being down. This problem is resolved after the host is brought back into service.
