2. fixed checkbox text
3. fixed messages displayed
4. renamed state blocked to hold and page blocked.html to hold.html
5. renamed title Annual Recertification to Annual Certification
6. formatted HTML pages by adding breakers where necessary
7. changed documentation link for all pages
