Feat update ui (pre react)

• Update the form to display shib user's shib attributes
• Remove old inputs for form
• Include cancel button on form
• Include customizable welcome message
• Update Cheaha logo
• Update backend to gather user's shib attributes
• Validate form prior to account request
• Pre-populate form with user attributes

Note: Complete features have yet to be tested in an Openstack Environment
